Speak with a provider before committing. Most patients skip this and proceed directly to the evaluation — but it’s available if you want it.

What happens in the consultation

30 minutes, in-person or Zoom. You’ll meet one-on-one with a provider, hear how the OHM approach differs from standard care, and ask whatever you need to ask. You’re welcome to bring any outside labs you already have.

Registration is required first

Because the consultation is a documented visit, you’ll need to register and complete a brief pre-consult intake beforehand. This lets the practice create a chart and have your basic information on file.

Step One

Complete Your Intake

Once you decide to move forward, you’ll be registered and given access to the patient portal to complete your intake forms.

What’s in the intake forms

A detailed health history covering your medical background, symptoms, goals, and lifestyle. Takes about 40–45 minutes. The provider uses your answers to personalize the evaluation before you arrive — including which specific lab markers to order.

Patient portal access

You’ll receive a portal access link by email after registering. Check your spam folder — these emails frequently land there. If you’re enrolling as a couple, each person completes their own forms independently.

Step Two

Get Your Blood Work Done

After submitting your intake forms, you’ll receive blood draw instructions by email. The lab panel covers 100+ biomarkers — well beyond what standard testing includes.

Where blood is drawn

Quest Diagnostics location near you, or an independent mobile phlebotomist who comes to your home. All analysis is performed at Cleveland Heart Lab, a specialized laboratory within the Quest network that runs advanced panels beyond standard Quest or LabCorp testing.

Insurance billing is available, but final cost cannot be guaranteed. Most patients choose self-pay for cost predictability.

Timeline from blood draw to evaluation

Expect roughly 2–3 weeks from your blood draw to your evaluation appointment. That window covers Cleveland Heart Lab processing (currently 5–7 business days) and provider scheduling availability. Scheduling begins as soon as your draw is confirmed, since appointments can book 3–4 weeks out.

Your Comprehensive Evaluation

The clinical entry point of the program. Four components, one visit. You leave with a complete picture of your health and a personalized plan.

— DEXA Scan — full body composition analysis: bone density, lean muscle, body fat, and visceral fat

— Cognitive Assessment — structured evaluation of memory, attention, and focus

— Lab Review — one-on-one with your provider, walking through every biomarker in plain language

— Personalized Plan — hormone therapy, nutrition, lifestyle, and long-term prevention strategy

Duration and pricing

Solo (3–4 hrs) $495 / person

Couples (4–5 hrs) $395 / person

Couples are seen simultaneously and save $200 total compared to two solo visits. Illinois pricing differs — ask for details.

Make Your Decision

After the evaluation, you have a complete health picture and a personalized plan in hand. Enrolling in the ongoing program is your choice — there’s no obligation.

If you enroll

You join the Precision Program at $299/month. The practice initiates your enrollment and places you on a bloodwork cycle. An 8-month minimum commitment is required — meaningful, measurable health changes take time.

If you don’t enroll

You keep your evaluation results and personalized plan. No ongoing obligation. If you want to revisit enrollment later, you can reinitiate at any time.

The Precision Program

Ongoing physician oversight, repeat testing, and direct provider access — all built around keeping your health moving in the right direction.

How ongoing care works

The $299/month covers ongoing oversight — not a mandatory monthly in-person visit. Your provider reviews your labs proactively and reaches out when adjustments are needed.

You have direct text access to the provider team for urgent concerns. Typical response time is within a few hours on weekdays.

No prescription or refill is issued without a current lab review first.

Bloodwork cycle

Once enrolled, you’re placed on a recurring bloodwork cycle — a core part of the program. Most patients complete labs 2–4 times per year, individualized based on health status and treatment plan.

Established patients use the limited follow-up panel ($399 self-pay) rather than the full initial comprehensive panel.

8-Month Minimum Commitment

Enrollment requires a minimum 8-month commitment. After the initial 8 months, continuation is based on your individual health goals.

Complements, Does Not Replace, Primary Care

Patients should maintain a primary care provider for preventive screenings and other primary care needs outside the OHM program’s scope.

Cash-Pay Practice

All clinic visits are cash-pay. Insurance is not accepted for visit fees. Blood work can be billed to insurance or paid at self-pay rates.

Common Questions

Is this just testosterone?

The OHM program is not a testosterone clinic. Hormone optimization — when appropriate — is one component of a broader clinical program that also includes metabolic health, cardiovascular risk, body composition, cognitive function, and long-term disease prevention. Testosterone may or may not be part of your personalized plan, depending on what your data shows.

Is this evidence-based or alternative medicine?

Evidence-based. Every recommendation is grounded in clinical research and measurable data from your own biology. The OHM approach uses advanced diagnostics and bioidentical hormones — not supplements, detoxes, or unvalidated protocols.

Do I have to stop seeing my regular doctor?

No. Patients in the Precision Program should maintain a primary care provider for preventive screenings, procedures, and other primary care needs. The OHM program is designed to run alongside primary care — not replace it.

What does it actually cost, start to finish?

Lab work ($599–$944 self-pay depending on panels ordered), the Comprehensive Initial Evaluation ($495 solo / $395 per person for couples), and the Precision Program at $299/month with an 8-month minimum commitment. The practice provides a personalized lab estimate before anything is ordered.

How is this different from the hormone clinic down the street?

Most hormone clinics focus narrowly on a single treatment — usually testosterone — without the diagnostic depth to understand what’s actually driving your symptoms. The OHM program evaluates 100+ biomarkers across hormones, metabolism, cardiovascular risk, body composition, and cognition before making any recommendations.

Will I be pressured?

No. After the evaluation, enrolling in the Precision Program is your decision. You leave with your results and your personalized plan regardless of whether you enroll. There’s no sales process and no follow-up pressure.

Do you accept insurance?

Insurance does not cover the type of personalized, preventive care provided through the OHM program. All clinic visits are cash-pay. Blood work can be billed to insurance — though coverage and out-of-pocket costs are determined by your carrier. Most patients choose self-pay for cost predictability.

Is this one-size-fits-all?

No. The lab panel ordered is partially determined by your intake forms. The personalized plan you leave the evaluation with is built around your specific biomarker results, medical history, and goals. Dosing is reviewed and potentially adjusted at every refill based on your current labs.
